• Wage statements (Form W-2) 
• Last year's tax return - NEW CLIENTS ONLY (Can't Find it? click here)
• Information on education expenses (tuition, loan information (including interest paid), any employer reimbursement)
• Cash and non-cash charitable donations
• Mortgage or home equity loan interest paid (Form 1098)
• Un-reimbursed employment-related expenses
• Job-related educational expenses
• Childcare expenses and provider information
• Real estate and personal property taxes
• Home Office (only if you use part of your home exclusivley for business - home office worksheet)
• Commissions received and/or paid
• Pension, or retirement income (Forms 1099-R)
• Information on the sale of stocks and/or bonds (Include a Cost Basis)
• Self-employed business income and expenses
• Lottery and/or gambling winnings and losses
• Social Security and/or unemployment income
• Income and expenses from rentals
• Alimony paid or received
• Record of purchase or sale of real estate
• Medical and dental expenses (must  exceed 7.5% of your AGI)
• Estimated taxes or foreign taxes paid
